How a referral works with Ava
- You send the basicsFamily name and phone, child’s age, Medicaid status if known, and the ordering physician. Call or fax, whichever is easier.
- We reach the familyWe call the family within one business day and explain GAPP in plain words.
- We gather the paperworkWith the family’s permission, we request the physician’s order and records and prepare the GAPP application with them.
- We keep you in the loopWith the family’s OK, we update you when they’re contacted, when the application is submitted and when care starts.

Will you tell me what happened with my referral?
Yes. With the family’s permission, we let you know when we’ve reached them, when the application is submitted and when care starts.
Can Ava take a referral before hospital discharge?
Yes, and earlier is better. Starting before discharge gives time to gather orders and records so care can be lined up for the move home.
Do you have handouts we can give families?
