What the RN does
- Assesses your child at home at the start of care.
- Writes the nursing care plan from your doctor’s orders.
- Updates the plan when orders, equipment or needs change.
- Supervises the LPNs on your child’s case.
- Takes RN-level shifts when your child’s needs call for them.
What you still do
You tell the RN what normal looks like for your child, what worries you, and what changed since the last visit. You keep making the parenting calls. The RN’s job is to turn the doctor’s orders into a plan every nurse on your case follows the same way.
When a child usually needs RN-level care
This is general guidance; the doctor’s order and GAPP decide.
- Care that changes often and needs a nurse to assess and adjust within the orders.
- Newly placed equipment, like a new trach or ventilator, while the routine settles.
- Right after a hospital stay. See hospital-to-home transition.
Have this ready for the first call
- Your child’s age, diagnosis and Medicaid status (or that you’re applying).
- The name and phone number of the doctor who manages your child’s care.
- A rough picture of a normal day and a hard day.
- Your current discharge papers or most recent orders, if you have them.

RN home care questions
Does my child need an RN or an LPN at home?
Your child’s doctor and GAPP decide. Children with the most complex or changing needs may need RN-level shifts; many others have an RN for assessment and supervision and LPNs for most shifts.
How often does the RN visit if my child has LPN shifts?
The supervising RN visits on a schedule set by GAPP rules and your child’s plan, and more often when something changes. Ask us what the schedule looks like for your case.
Who writes my child’s home care plan?
An RN writes the nursing care plan from your doctor’s orders and an assessment of your child at home, and updates it when orders or needs change.
