A pediatric LPN gives skilled nursing care at home on scheduled shifts, following the RN’s care plan and your doctor’s orders. For most families on GAPP nursing, the LPN is the nurse in the house day to day.

What an LPN does on shift
- Gives treatments, feeds and medicines on schedule.
- Cares for airway and equipment as the plan says.
- Watches for changes and follows the emergency plan.
- Writes down what happened on the shift.
- Hands off to you or the next nurse.
What stays with the RN
The LPN doesn’t rewrite the plan. If something changes, the LPN reports it to the supervising RN and your doctor, and the RN updates the plan.
Making LPN shifts work in your home
- Space
The nurse needs a spot near your child for supplies and notes. It doesn’t have to be a separate room.
- Handoffs
Five minutes at the start and end of each shift saves hours of confusion. Tell the nurse what changed.
- Night shifts
Many families use LPN hours overnight so a parent can sleep. Ask us how that works for your approved hours.
Have this ready for the first call
- Your child’s age, diagnosis and Medicaid status (or that you’re applying).
- The name and phone number of the doctor who manages your child’s care.
- A rough picture of a normal day and a hard day.

- Who supervises the nurses on my child’s case, and how do I reach them?
- What happens if a nurse calls out? Will you tell me before the shift?
- How do you train nurses on my child’s specific equipment and orders?
- Can I meet or talk with a nurse before the first shift?
LPN home care questions
Can an LPN care for a child with a trach at home?
Often yes, when the doctor’s orders and care plan allow it and the LPN is trained on your child’s equipment. The supervising RN sets the plan the LPN follows.
Is an LPN a real nurse?
Yes. A Licensed Practical Nurse is licensed by the state to give nursing care. At home, LPNs work under an RN’s care plan and your doctor’s orders.
What happens if our regular LPN is sick?
We tell you as early as we can, try to send a nurse who already knows your child, and keep you updated if a shift can’t be covered.
